ONLINE PASSPORT APPLICATION
ALL PASSPORTS ISSUED TO AN ADULT AFTER SEPTEMBER 2001 MAY BE RENEWED VIA THE ONLINE PROCESS AT www.pica.gov.jm. IF ALL YOUR INFORMATION REMAINS THE SAME AS IN THE PASSPORT YOU ARE RENEWING. THIS PROCESS IS SAFE AND QUICKER THAN THROUGH THE CONSULATE.

Please note professional photography service is available on the building at 44 W. Flagler should there be any challenge accessing this service before arrival at our office.
First Time Applicants: Adults
- Completed passport application form (Certified at Section G)
- Completed “First Time Application Form”
- Two (2) identical non digitized color photographs, ( One photograph must be certified by the same official who certified the passport application)
- Original Birth Certificate or Adoption Register and letter advising how you arrived in the United States (for persons born in Jamaica) or Citizenship Certificate for persons born overseas
- Proof of name change. If name has been changed, submit either ( Original Marriage Certificate, Original Deed Poll, Original Divorce Decree or Court Order)
- Government issued ID such as Driver’s Licence or Passport
- All first-time applicants must apply in person
FEE: U$120.00 – First Time Adult (Payable by Money Order to: Consulate of Jamaica, Miami – Personal Cheques are not acceptable)
Adult Passport Renewal
- Completed passport application form (Certified at Section G)
- Current Passport
- Two (2) identical non digitized color photographs, ( One photograph must be certified by the same official who certified the passport application).
- Original Birth Certificate / Adoption Certificate or Original Jamaican Citizenship ( as applicable )
- Proof of name change. If name has been changed, submit either ( Original Marriage Certificate, Original Deed Poll, Original Divorce Decree or Court Order).
- Notarised copy of valid Government issued Photo Identification (Example: Drivers Licence, Voter Identification) – If visiting the Consulate please take the original document
- An applicant whose last passport was issued as a notarised copy of valid Government issued Photo Identification (Example: Drivers Licence, Voter Identification) If the applicant does not have an ID, the parent should complete a Declaration of Identity Form and submit along with a notarised copy of their ID. (If visiting the Consulate please take the original document)

Adult Passport Replacement ( Lost or Stolen Passport )
- Completed passport application form (Certified at Section G)
- Police Report or Lost Passport Form and Lost Passport Form Two
- Two (2) identical non digitized color photographs, ( One photograph must be certified by the same official who certified the passport application).
- Original Birth Certificate / Adoption Certificate or Original Jamaican Citizenship ( as applicable )
- Proof of name change. If name has been changed, submit either ( Original Marriage Certificate, Original Deed Poll, Original Divorce Decree or Court Order ).
- Notarised copy of valid Government issued Photo Identification (Example: Drivers Licence, Voter Identification) – If visiting the Consulate please take the original document
FEE: U$200.00 – Adult Replacement (Payable by Money Order to: Consulate of Jamaica, Miami – Personal Cheques are not acceptable)
Processing time for passport applications: Approximately 6 weeks for in person applications; approximately 3 months for applications received by mail
Applicants submitting their own applications at the Consulate are not required to have forms and photographs notarized.
