Passport Renewals and First Time Applications
Adult Renewal – $93.00 – ($106.00 inclusive of processing and mailing fees)
Minor Renewal – $63.00 – ($76.00 inclusive of processing and mailing fees)
Lost/Stolen/Damaged/Destroyed Passport Applications
Adult Replacement – $173.00 – ($186.00 inclusive of processing and mailing fees)
Minor Replacement – $113.00 – ($126.00 inclusive of processing and mailing fees)
If you wish your new passport and the original documents submitted along with your application to be returned to you, without paying the mailing fees, you have the option to:
- Send a self-addressed stamped envelope.
The payment for passports are to be in the form of Money Order/Bank Draft made payable to the Jamaican High Commission in the relevant amounts. (Do not send cash in the mail).

Guidelines on how to properly complete the Passport Application Form:
- Use the most recent passport in your possession along with the original certificates/documents to complete the application form.
- Ensure information is consistent throughout the documents such as dates and the spelling of all names.
- Ensure the applicant’s signature is affixed within the rectangular box, which sits in the middle of two square boxes at the top of page 2. The signature must not touch any of the lines of the rectangular box. Minors ages 12 and over are required to affix their signature on the application form. They are not required to sign anywhere else on the form. (Minors under the age of 12 are not required to affix signatures).
- The signature in the rectangular box must match the declarant’s signature in Section E at the bottom of page 2, with the exception of minors under the age of 12. If the signatures are not consistent submit a blank typing paper with four (4) samples of applicant’s signature.
- The parent giving consent for the child to hold a passport will complete and sign Sections C and E of the passport application form.

Adults
- Completed Passport Application Form.
- Original passport (current or expired).
- Original Birth Certificate, Certificate of Citizenship or Adoption Register.
- Two (2) colour non-digitized passport size photographs; one of which should be certified (including mailed-in-applications). See the passport picture specifications and passport photo requirements
- A Canadian Deed Poll or Deed Poll from the Registrar General’s Department in Jamaica or other relevant authority in the country of residence, must be submitted where there has been a change of name.
- Section G of the passport application form must be certified by one of the following persons:
- A Consular Officer at the Jamaican High Commission in Ottawa or the Consulate General in Toronto.
- A Jamaican Honorary Consul in Canada.
- An Attorney-at-law.
- A Notary Public.
- Minister of Religion authorized to perform marriages.
- A Medical Doctor.
- Married women must submit a Marriage Certificate with their application, if they are using their married name. Women who are divorced and who wish to use their married name should submit the Divorce Judgement. All persons who have been married are required to complete Section B of the Passport Application Form.

Minors
- Completed Passport Application Form.
- Original Passport (current or expired).
- Original Birth Certificate, Certificate of Jamaican Citizenship or Adoption Register. The name of the parent providing consent for the minor to hold a passport must appear on the minor’s Birth Certificate at the time of application.
- Two (2) colour non-digitized passport size photographs; one of which should be certified (including mailed-in-applications). See the passport picture specifications.
- Valid identification such as a passport, driver’s licence or other government issued ID in the country of residence of the parent providing consent for the minor to hold a passport.
- Section G of the passport application form must be certified by one of the following persons:
- A Consular Officer at the Jamaican High Commission in Ottawa or the Consulate General in Toronto.
- A Jamaican Honorary Consul in Canada.
- An Attorney-at-law.

- Legal Guardians are required to submit relevant documentation showing legal guardianship along with a valid ID or a notarised letter of consent from the parent(s) and a copy of the parent’s valid identification such as a passport, driver’s licence or other government issued ID in the country of residence.

Adults

- Marriage Certificate or Divorce Judgement, for married women, if the application is being made in the married name.
- Photographic identification such as driver’s licence, cancelled Jamaican passport, other passport from country of citizenship, identification indicating permanent residency and other government issued identification, subject to the approval of the Consular Officer.
- Police Report including the Incident Number– this document is mandatory for lost/stolen passports.
- Two (2) colour non-digitized passport size photographs; one of which should be certified (including mailed-in-applications). See the passport picture specifications.
- Section G of the passport application form must be certified by one of the following persons:

- Parents are to appear in person at the Jamaican High Commission in Ottawa, the Consulate General in Toronto or any of the Honorary Consulates in Canada, with children 3 years and older. If mother is married, the Marriage Certificate is required.
- Legal Guardians are required to submit relevant documentation showing legal guardianship along with a valid ID or a notarised letter of consent from the parent(s) and a copy of the parent’s valid identification such as a passport, driver’s licence or other government issued ID in the country of residence.
Adults

Minors
